Washington \u00a0Nephrology \u00a0Associates \u00a0(WNA) will use reasonable means \u00a0to protect \u00a0the privacy of your health information \u00a0sent \u00a0by email. \u00a0However, \u00a0because of the risks outlined \u00a0below WNA \u00a0cannot guarantee \u00a0that email communications will be confidential.\u00a0 \u00a0Additionally, WNA will not \u00a0be liable in the event \u00a0that you or anyone else \u00a0inappropriately\u00a0 \u00a0uses \u00a0your \u00a0email.\u00a0\u00a0 \u00a0WNA \u00a0will not \u00a0be \u00a0liable \u00a0for \u00a0improper \u00a0disclosure \u00a0of \u00a0your \u00a0health information that is not caused by WNA’s intentional misconduct.<\/p>\n

Email risks \u00a0and \u00a0your \u00a0responsibility<\/h2>\n

At the discretion of the WNA, its staff, physicians and agents (WNA) and upon \u00a0your agreement \u00a0to the terms outlined within this consent \u00a0form, you may use email to communicate \u00a0with WNA. \u00a0These emails may contain your personal \u00a0health information.\u00a0 \u00a0If you decide \u00a0to use email to communicate with \u00a0WNA, you should \u00a0be aware of the following risks and\/or your responsibilities:<\/p>\n

    \n
  1. \u00a0 As the \u00a0Internet is not \u00a0secure \u00a0or \u00a0private, \u00a0unauthorized \u00a0people \u00a0may \u00a0be able \u00a0to intercept, read and possibly modify email you send or are sent by WNA.<\/li>\n
  2. \u00a0 You \u00a0must \u00a0protect\u00a0 \u00a0your \u00a0email account, \u00a0password \u00a0and \u00a0computer \u00a0against \u00a0access \u00a0by \u00a0unauthorized people.<\/li>\n
  3. \u00a0 Since email can used to spread viruses, some which cause email messages to be sent to people who you \u00a0do \u00a0not \u00a0intend\u00a0 \u00a0to send \u00a0email messages \u00a0to, \u00a0you\u00a0 should \u00a0install \u00a0and \u00a0maintain \u00a0virus \u00a0protection software on your computer.<\/li>\n
  4. \u00a0 Since emails can be copied, printed and forwarded \u00a0by people to whom \u00a0you send emails, you should be careful regarding whom you send emails.<\/li>\n
  5. \u00a0 As your employer \u00a0may claim ownership of, or the right to access, the email account \u00a0issued to you by our email, you should avoid using an employer issued email account \u00a0to communicate with WNA.<\/li>\n<\/ol>\n

    Conditions for the use of email<\/h2>\n

     <\/p>\n

    By consenting to the use of email with WNA, you agree that:<\/p>\n

    \u00a0WNA may forward emails as appropriate for diagnosis, treatment, reimbursement, and other \u00a0related reasons. \u00a0As such, WNA staff members, other \u00a0than the recipient, may have access to emails that you send. \u00a0Such \u00a0access will only \u00a0be to such \u00a0persons \u00a0who \u00a0have a right \u00a0to access \u00a0your\u00a0 email to provide services \u00a0to you.\u00a0 \u00a0Otherwise, \u00a0WNA will not, \u00a0otherwise, forward \u00a0emails to independent third parties without \u00a0your prior written consent, except as authorized or required by law.<\/span><\/p>\n

    1. \u00a0Although \u00a0WNA will try to read and respond promptly to your emails, WNA staff may not read your email immediately.\u00a0 \u00a0Therefore, you should \u00a0not \u00a0use email to communicate with WNA \u00a0if there is an emergency or where you require an answer in a short period of time.<\/li>\n
    2. \u00a0 If your \u00a0email requires \u00a0or \u00a0asks \u00a0for \u00a0a \u00a0response, \u00a0and \u00a0you \u00a0have \u00a0not \u00a0received \u00a0a \u00a0response \u00a0within \u00a0a reasonable \u00a0time period, it is your responsibility to follow up directly\u00a0with WNA.<\/li>\n
    3. \u00a0You \u00a0should carefully consider \u00a0the risk of using email for \u00a0the communication of \u00a0sensitive \u00a0medical<\/li>\n
    4. Information, \u00a0such\u00a0 \u00a0as, \u00a0but\u00a0 \u00a0not \u00a0limited \u00a0to, \u00a0information\u00a0 \u00a0regarding\u00a0 \u00a0sexually \u00a0transmitted\u00a0 \u00a0diseases, AIDS\/HIV, mental health, developmental disability, or substance abuse.<\/li>\n
    5. \u00a0You \u00a0should \u00a0carefully \u00a0word \u00a0your \u00a0email messages \u00a0so \u00a0the information provided \u00a0clearly, yet briefly,<\/li>\n
    6. \u00a0Describes the information you intend to convey. \u00a0You should avoid writing long “chatty” emails.<\/li>\n
    7. \u00a0You are responsible for correcting any unclear or incorrect information.<\/li>\n
    8. \u00a0WNA \u00a0reserves the right \u00a0to save your email and include your email or information contained \u00a0within your email in your medical record.<\/li>\n
    9. It \u00a0is \u00a0the \u00a0patient’s\u00a0 \u00a0responsibility\u00a0 \u00a0to \u00a0follow \u00a0up \u00a0and\/or schedule \u00a0an \u00a0appointment if \u00a0warranted \u00a0or recommended by WNA.<\/li>\n
    10. Emails may not \u00a0be the only form of communication that WNA \u00a0will use to communicate \u00a0with you.<\/li>\n
    11. Additionally, WNA \u00a0may decide that it is not in your best interest \u00a0to continue \u00a0to communicate \u00a0with you by email. In such case, WNA will notify that it no longer intends \u00a0to communicate \u00a0with you by email.<\/li>\n<\/ol>\n
