To request an appointment as a new patient, please submit the\u00a0online Registration Form<\/a><\/strong>. Once received, you will be contacted for scheduling the appointment. Feel free to call one of Our Offices\u00a0<\/a>directly for any questions.<\/p>\n Be sure to have your insurance information with you when you call. Our office will verify your insurance before the first visit. If there is a problem we will contact you. If your insurance plan requires a referral from your primary care provider, we will let you know.<\/p>\n Please make sure to fill out the online\u00a0Registration Form<\/a>. You can also download and print the form here<\/a>. <\/strong><\/p>\n We will ask you for demographic information. You will sign a consent, acknowledgement forms, and an authorization for release of personal health information. You will also review our Notice of Privacy Practices. If you would like a copy of the Notice of Privacy Practices, please ask for one.<\/p>\n To your first visit please be sure to bring:<\/p>\n If you must cancel a scheduled appointment, please notify us at least 48 hours<\/strong> in advance.<\/p>\n<\/div>\n<\/div>\n Please contact your office<\/a> directly for office hours.<\/p>\n<\/div>\n<\/div>\n Lab Testing and Test Results<\/span>\u00a0<\/span><\/b><\/p>\n<\/div>\n<\/div>\n Many of our physicians will request that you come to the office before your scheduled appointment to have laboratory testing so that the report is available to discuss with you at the time of your appointment with the physician.
