Request an Appointment

To request an appointment as a new patient, please submit the online Registration Form. Once received, you will be contacted for scheduling the appointment. Fee free to call one of Our Offices directly for any questions.

Be sure to have your insurance information with you when you call. Our office will verify your insurance before the first visit. If there is a problem we will contact you. If your insurance plan requires a referral from your primary care provider, we will let you know.

New Patients

Please make sure to fill out the online Registration Form prior to the visit in order to save time. You can also download and print the form here.

If the form is not completed prior to the visit, please plan to arrive 30 minutes before your scheduled appointment time to complete paperwork. We will ask you for demographic information. You will sign a consent, acknowledgement forms, and an authorization for release of personal health information. You will also review our Notice of Privacy Practices. If you would like a copy of the Notice of Privacy Practices, please ask for one.

To your first visit please be sure to bring:

  • Insurance Card
  • Drivers License or Photo Identification Card
  • Completed health history which is part of the registration form
  • All Current Medications you are taking. Bring the medications rather than just a list of them.

Cancel an Appointment

If you must cancel a scheduled appointment, please notify us at least 48 hours in advance.

Lab Testing and Test Results 

Many of our physicians will request that you come to the office before your scheduled appointment to have laboratory testing so that the report is available to discuss with you at the time of your appointment with the physician. You may be directed to a laboratory draw station if it is more convenient for you.

We encourage you to register on our Patient Portal to receive lab and test results more quickly.
